Where do I fill out my name and certification number for BACB® credit?

We need your first name, last name and BACB® certification number exactly as it appears on the BACB® site. However you enter it here is how it will appear on your certificate. If there is a discrepancy, the BACB® may not accept the certificate for credit. You can verify if this is correct or enter your details below.
